Our Story
The Hoosier Crescent Foundation (HCF) was founded by concerned American Muslim citizens who recognized a critical need: many Indiana residents struggle to access healthcare and are unaware of the resources available to them. HCF was established to bridge this gap by providing free, accessible medical care to uninsured and underserved adults in our community.
Our goal is to improve health outcomes while reducing the burden on emergency departments, which are often used for non-emergency and primary care by those with no other options. Through our clinic and outreach efforts, we strive to offer not only medical services, but also dignity, education, and support to every patient we serve.
HCF’s mission is inspired by the Islamic teachings and its emphasis on compassion, justice, and service to others. One verse from the Qur’an captures this spirit well:
“Whoever saves a life, it is as if they had saved all of humanity.”
(Qur’an 5:32)
With this guiding principle, we are committed to building a healthier, more equitable future—one patient at a time.

Our Mission
Our mission is to provide free medical services to uninsured adults—regardless of age, race, ethnicity, national origin, gender, disability, sexual orientation, political affiliation, or religious belief. We are dedicated to delivering care in a manner that reflects the values of compassion, equity, and service at the heart of both Islamic ethics and American ideals.

Dr. Sajjad HussainChief Medical Officer (CMO)
Sajjad Hussein has been a resident of Indiana since 1991 and currently lives in Zionsville. He and his wife have three children. He is a chief of vascular heart surgery at St. Vincent Heart Center, since 2008. Sajjad is an avid golfer and squash player. |

Mohamad Heikal has been a resident of Indiana since 2013 and currently lives in Carmel. He and his wife have three children. He is a critical care physician at St. Vincent Hospital. |
Mahmood IqbalBoard Member
Mahmood Iqbal has been a resident of Indiana since 2006 and currently lives in Carmel. He and his wife have three children. He is the President and CEO of Home Healthcare Solutions. Mahmood is a passionate poet. |
Dr. Ahmad SaltagiBoard Member
Ahmad Saltagi has been a resident of Indiana since 2005 and currently lives in Carmel. He and his wife have seven children. He is at an internist. Ahmad has served on the board for many non-profit organizations. |
Dr. Waseem ZahoorBoard Member
Waseem Zahoor has been a resident of Indiana since 2000 and currently lives in Carmel. He is an associate Chief of Staff-Chief of Primary Care at a Veterans Affairs Medical Center, Chief Hospitalist at an academic Veterans Affairs Medical Center and a President of a private medical practice. He is passionate about social welfare and has served on Boards for local social organizations. |

Bylaws
The corporation is organized as a not-for-profit corporation under Indiana Statutes, incorporated on a non-stock basis. The purpose for which the corporation is formed is exclusively for charitable and educational purposes within the meaning of Section 501(c)(3) of the Internal Revenue Code of 1986, as amended, or the corresponding provision of any future United States Internal Revenue law.
